Be sure to have the St. Lucy's clearance forms signed and returned to Mrs. Torres by July 31, 2014. Additional copies are available in room 501, but you are able to print and sign by selecting the following links:

- Including Doctor Release Form (page 1, 2) which requires the Doctor's office stamp on the top right of the page.
